Abstract

Context-aware care navigation systems use real-time context information and service-oriented decision-support systems to deliver personalized health and care services. While these systems offer considerable promise to address the growing complexity and fragmentation of healthcare delivery, their real-world implementation is often hindered by poor data quality and interoperability, insufficient reliability and explainability, and lack of scalability and sustainability. Addressing these challenges requires dedicated efforts on four interdependent fronts: (i) establishing the Foundations of Context-Aware Care Navigation; (ii) developing robust Knowledge Fusion technology, particularly within the domain of Medical Informatics; (iii) implementing practical Architectural Frameworks that enable the incremental deployment of real-world use-case scenarios; and (iv) designing and applying suitable Evaluation and Validation Strategies.

Health information is a key enabler of innovative context-aware solutions, particularly for the health and care subdomains. A multitude of organizations worldwide process and publish high-quality health information on a daily basis. However, such information is not always readily available when needed due to diverse factors, including the lack of established semantic interoperability between sources. Intelligent Knowledge Fusion technology can serve as a powerful means to mitigate this limitation.
